I am currently a patient at one of NUP polyclinics and would like to enrol with the polyclinic, but I have not received my SMS from MOH. Can I enrol now?

MOH is sending out the SMSes progressively, starting with those aged 60 years and above. Do wait to receive the SMS from MOH. Once you have received it, you may enrol on the HealthHub app using the link given and choose one of our polyclinics as your preferred clinic. We welcome our current polyclinic patients to enrol and continue your care with us.

Help! I want to enrol to one of NUP polyclinics but I do not have a smart phone and I need help enrolling.

You may visit any of our polyclinics (Bukit Batok, Bukit Panjang, Choa Chu Kang, Clementi, Jurong, Pioneer, Queenstown) for assistance. Our patient ambassadors will answer any of your Healthier SG-related queries.

I would like to enrol to a Healthier SG-registered GP clinic or polyclinic that is not part of the National University Polyclinics. Can the polyclinic staff assist me with enrolment?

Assisted enrolment is also available at Community Clubs (CCs) and Joint Testing & Vaccination Centres (JTVCs). You may wish to visit a nearby CC or JTVC for help with enrolling to the Healthier SG-registered GP clinic or polyclinic.

I am already enrolled to an NUP Polyclinic A but I want to have my Health Plan discussion at NUP Polyclinic B. Is that possible?

Unfortunately, you are required to attend and complete your first visit health plan discussion at the clinic that you have enrolled to. If you wish to change the clinic, you will be able to do so through the HealthHub app, limited to certain terms and conditions. Please visit healthiersg.gov.sg for more information.
