Under this model, you will be managed by a core team, comprising family physicians, a care coordinator and a care manager, to journey with you and your family as we aim to keep your conditions under control. You will be educated by the team on healthier lifestyle choices, closely monitored with regard to your disease control, and actively engaged in the decision-making process around your individually-tailored care plan. The core team is closely supported by pharmacists and allied health professionals such as dietitians, medical social workers, physiotherapists, podiatrists and psychologists.
