Care for Chronic Medical Conditions

By 2050, nearly 1 million Singaporeans could be affected by diabetes1. The National Population Health Survey 2020 revealed an increase in the prevalence of high blood cholesterol and high blood pressure among Singaporeans2. The rising prevalence of chronic diseases in Singapore is due to the country's ageing population and adoption of unhealthy lifestyles. With age, the likelihood of developing one or more chronic diseases such as diabetes, high blood pressure and high blood cholesterol increases. Early detection and appropriate management of such diseases can delay or avoid the onset of complications such as stroke and heart diseases, as well as the significant medical expenditures associated with hospitalisation.

Patients with the above-mentioned chronic diseases make up a significant percentage of those who visit the polyclinics. At the National University Polyclinics (NUP), we focus on health promotion, disease prevention, and chronic diseases management. We believe the key to a healthier population begins with a strong primary care team.

Our multi-disciplinary care team comprises doctors, advanced practice nurses, care managers and coordinators as well as allied health professionals. Recommended Intervals of Reassessment for Diabetes, High Blood Pressure and High Blood Cholesterol Management

Patients with good diabetes, blood pressure, and cholesterol level control

Patients not on medication and without complications - Every 6 months.

Patients on medication, are elderly, have multiple complications such as stroke, heart disease or kidney impairment - Every 3 to 4 months.

Patients with adjustments to medications

Within 1 to 3 months.

Patients with diabetes and require insulin injections

Within 1 to 8 weeks.
