Request for Medical Report

A medical report can be requested

  • in person at the registration or payment counter at any of our polyclinics OR
  • by mailing the required documents to the respective polyclinics (attention to Service & Operations Department). Addresses can be found here.

Medical report payment can be made

  • in person at the payment counter at any of our polyclinics OR
  • using cheque for requests via mail. Cheque is to be crossed and made payable to "National University Polyclinics". Medisave cannot be used to pay for the medical report.
Required Documents
  • A copy of the patient's NRIC / identification documents
  • A copy of the completed application form for the release of medical information. Please download the form here.

    • For patients below 21 years old, consent of the patient's parents or legal guardian is required.
    • If the patient is deceased or unable to give consent, the consent from the appointed representative of patient's estate is required.
    • Other supporting documents including death certificate, birth certificate, marriage certificate or letter of administration as proof of representative's relationship to patient.
    • The validity of the consent given is six months. If the consent form was signed more than six months ago, the patient / parents / legal guardian will need to sign a new application form.

Processing Time

The time required to process a medical report request is approximately four weeks from the date we receive the application. It may take longer in the event that:

  • The application form is incomplete or supporting documents required for verification are not produced.
  • Verification of the request purpose is required.
  • The specific doctor required to process the medical report is not available.
  • Access to the patient's medical information is blocked due to unforeseen circumstances.
Collection of Medical Report
  • Patients can collect the report personally at the respective polyclinics. The patient's contact number is to be provided at the point of application. Patients will be contacted once the report is ready for collection.
  • Patients will be able to request for the medical report to be sent to them through registered mail (with additional fee of $4.91), courier (with additional fee of $7.41) or email. The email or mailing address is to be provided at the point of application.
  • If a third party is collecting the report for the patient, an authorisation letter is required. Please download the form here.
For insurance claim request or employers requesting for medical reports of foreign workers
