I am currently a patient at one of NUP polyclinics and would like to enrol with the polyclinic, but I have not received my SMS from MOH. Can I enrol now?
MOH has invited Singapore Citizens and Permanent Residents aged 40 and above via SMS to enrol with a Healthier SG clinic or polyclinic. SMS invitations will continue to be sent to residents when they turn 40 years old. If you are 40 and above, you can self-enrol using the HealthHub app or enrol at any of the Enrolment Stations (see https://gowhere.gov.sg/healthiersg for locations) without the need of an SMS invitation.
Help! I want to enrol to one of NUP polyclinics but I do not have a smart phone and I need help enrolling.

You may visit any of our polyclinics (Bukit Batok, Bukit Panjang, Choa Chu Kang, Clementi, Jurong, Pioneer, Queenstown) for assistance. Our patient ambassadors will answer any of your Healthier SG-related queries.

I would like to enrol to a Healthier SG-registered GP clinic or polyclinic that is not part of the National University Polyclinics. Can the polyclinic staff assist me with enrolment?

Assisted enrolment is available at Community Clubs (CCs) and Active Aging Centres (AACs). You may wish to visit a nearby CC or AAC for help in enrolling to the Healthier SG registered GP clinic or Polyclinic.

I am already enrolled to an NUP Polyclinic A but I want to have my Health Plan discussion at NUP Polyclinic B. Is that possible?

Unfortunately, you are required to attend and complete your first visit health plan discussion at the clinic that you have enrolled to. If you wish to change the clinic, you will be able to do so through the HealthHub app, limited to certain terms and conditions. Please visit healthiersg.gov.sg for more information.

Can I enrol with a polyclinic instead of a GP clinic? I have a chronic illness and I've been visiting my polyclinic regularly for my chronic treatment and medication.

You have the choice to select your preferred primary care provider under Healthier SG. We encourage you to enrol with the clinic that you go to frequently for your chronic treatment.

For continuity of care, we strongly encourage you to choose your regular polyclinic or Healthier SG GP clinic and commit to one Healthier SG clinic for your care needs. This will help you develop a long and trusted relationship with your care provider, who will serve as your first point-of-contact to holistically manage your health. You can be assured of consistent standards of care as GP clinics and polyclinics are both guided by the same care protocols.

Can I still visit the polyclinic for routine checkups if I register with a GP?
Healthier SG benefits are available/ accessible at your enrolled Healthier SG clinic. If you choose to enrol in a clinic that is different from where you get treated for your chronic condition currently, and you have pre-existing appointments, please reschedule your appointments to your newly enrolled Healthier SG clinic to enjoy the benefits of Healthier SG.
How often do I need to visit my Healthier SG clinic?

Your clinic will advise you on your next check-in dates.

Those with chronic conditions will typically require two to four follow-up consultations annually with a doctor. Prevailing subsidies, such as the Community Health Assist Scheme (CHAS), will apply for visits and treatment. The enhanced Healthier SG benefits will apply for residents seeking care from their enrolled clinic.

For those without chronic conditions, you may check-in with your enrolled clinic. A physical consult must be done at least once every three years.
